The Carver Department of Health is scheduled to receive the vaccine for H1N1 virus. The school department is working with the health department to schedule clinics.

The second clinic will be for students in grades 6-12.

DATE: Monday, December 7, 2009
TIME: 4:00 pm to 7:00 pm
PLACE: Carver Middle/High School Cafeteria

IMPORTANT: Please enter the cafeteria through the rear doors at the back of the building.

Parents will need to bring a completed copy of the consent form to the clinic (you may click on the link below to download a copy of the form). There is no fee for this vaccination.

As more vaccination is received, more clinics will be scheduled for other age groups. The Carver Department of Health is following the Massachusetts Department of Health recommended priority list for all clinics.
