WELCOME

Welcome to the Carver Middle High School, School Psychologist webpage.  Allow me to introduce myself and explain my role within the school.  My name is Melissa Gray and I am Carver Middle High School's School Psychologist.  As the School Psychologist, I conduct psychological/cognitive testing on students to assess learning style, strengths and weaknesses, and social-emotional functioning (if this is the area of need).  My results in conjunction with achievement testing and other areas (if needed) are presented at team meetings, in which the team determines if a student is found eligible for special education services.  At the team meeting, I also serve as Team Chairperson.
 

HOW DO I REQUEST THAT MY CHILD BE TESTED FOR SPECIAL EDUCATION?

HELPFUL FORMS 

  • At an eligibility meeting, (either an Initial or 3-year Re-Evaluation), the following agenda is used.  Additionally at this eligibility meeting, the team will determine eligibility by completing the Eligibility Flow Chart.
  • If my son/daughter is on an IEP, his/her IEP will be reviewed every year at the Annual Review meeting.  The following agenda is used.
  • At a Transfer IEP Meeting (when a student transfers to Carver who was previously on an IEP at their prior school), the following agenda is used.
